Spartan Health Surgicenter is regulated by the State and Federal laws that require us to maintain the privacy of health information protected those laws and to provide you with notice of our legal duties and our privacy practices with the respect to protecting your health information.

EOE

The Spartan Health Surgicenter facility agrees to comply with the provisions of the Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973 and the Age Discrimination Act of 1975, Americans with Disabilities Act, and all requirements imposed by these acts and any other applicable regulations or standards, to the end that no person shall on the grounds of race, color, religious creed, ancestry, national origin, age or sex be denied benefits or services.
