Patient Information

GENERAL INFORMATION
In order to make your visit more comfortable, please:

  • Wear loose clothing and low-heeled shoes.
  • Leave all valuables and jewelry at home.
  • Remove contact lenses, make-up, nail polish, body piercings and metal hair clips before arriving.

WE MUST EMPHASIZE THAT YOUR COMPLIANCE WITH THE FOLLOWING PREREQUISITES IS MANDATORY TO AVOID DELAYS OR POSSIBLE CANCEL­LATION OF YOUR OPERATION.

1.       DO NOT EAT OR DRINK ANYTHING (NOT EVEN COFFEE, TEA, WATER, CHEWING GUM, CANDY OR MINTS) AFTER MID­NIGHT THE EVENING BEFORE SURGERY. THIS IS IMPORTANT TO PREVENT VOMIT­ING DURING SURGERY WHICH COULD HAVE SERIOUS CONSEQUENCES FOR YOU. SIMILARLY, TAKE NO MEDICATIONS UNLESS SO INSTRUCTED.

2.       SHOULD YOU DEVELOP A COLD OR OTHER ILLNESS BEFORE THE DAY OF SURGERY OR SHOULD YOU BE UNABLE TO KEEP YOUR DAY SURGERY APPOINT­MENT FOR ANY REASON, PLEASE CALL THE CENTER IMMEDIATELY SO THAT YOU MAY BE RESCHEDULED OR FURTHER EVALUATED.

3.       IF YOU ARE NOT HOME THE DAY PRIOR TO SURGERY, PLEASE CALL THE ANES­THESIA DEPARTMENT AFTER 1:00 PM FOR INSTRUCTIONS.

4.       PLEASE ARRIVE AT THE TIME INSTRUCT­ED BY THE SURGICENTER STAFF.

 

PREPARING FOR YOUR VISIT
Insurance Information - Please contact your insurance company to verify coverage for the planned surgery at Spartan Health Surgicenter. It is your responsibility to make sure all needed authorizations, referrals, and pre-testing results are received prior to surgery. Bring insurance cards and forms with you to give to the Spartan Health Surgicenter receptionist.
Patient Health History Form - Please complete the entire patient questionnaire about your health history. Be sure to bring this form when you arrive for your surgery. This information helps the doctors and nurses at Spartan Health Surgicenter give you the best possible care.
Pre-Operative Assessment Form
Pre-Operative Testing - The studies the doctor has ordered will be performed at a lab you or your doctor prefers. The tests should be scheduled so that results will be received at least two (2) days prior to your surgery. Bring the complete Patient Information Packet with you to your scheduled pre-operative testing appointment.
Pre-Anesthetic Instructions - If you are to receive anesthesia or intravenous sedation, one of the anesthesia staff members will call you before 1:00 PM the day before your surgery to give you specific pre-operative instructions and an approximate time of discharge. A nurse will call patients scheduled for local anesthesia cases. Our staff members will be happy to answer any questions you may have.
Dietary Restrictions - The evening before your procedure you may eat until 8 p.m. and have non-alcoholic liquids until midnight unless you have a special prep. Nothing - including water, chewing gum, candy or mints - is permitted after midnight, unless instructed otherwise by the anesthesia department. If you are to have a local anesthetic, please follow the same instructions.
Medications - You may take only the medica­tions that your surgeon or the anesthesia provider has instructed you to take. If you have any questions on what medications you should take the day of the surgery, please call the Anesthesia Department at 724-483-2760. These medication instructions will be reviewed with you the day prior to surgery. Please stop all herbal preparations, vitamins and aspirin products at least one week before surgery.

DAY OF YOUR VISIT
Arrival - Please arrive at the Spartan Health Surgicenter at your scheduled time as provided by the Anesthesia Department. This will allow us adequate time to prepare and start your procedure on time. If you have any problems with your scheduled arrival time or you need special arrangements, please call the Spartan Health Surgicenter as soon as possible.
Post-Anesthesia Recovery - If you receive any type of anesthesia, you will awaken in the Post-Anesthesia Care Unit. You will remain there until a member of the anesthesia staff approves your discharge.
Transportation Home - Please have a responsible adult escort you to and from the Spartan Health Surgicenter. You should not plan to drive or operate any equipment on the day of your procedure. Even if you receive a local anesthetic, we strongly encourage you to have an adult accompany you home. Your escort should plan on waiting for you during your stay.
Children - A parent or legal guardian must accompany all patients under the age of 18 and must remain at the Spartan Health Surgicenter while the patient is in the operating room. We encourage two adults to accompany young chil­dren. Clearance by your child's pediatrician may be required prior to surgery.
Post-Operative Instructions - You and your companion will receive post-operative instructions prior to your discharge from the Post-Anesthesia Care Unit. You will be contacted by a member of the Spartan Health Surgicenter staff within 48 hours following your visit to evaluate your recovery and to answer any questions you may have.

ADVANCED DIRECTIVES
We are required by law to request and provide information on living wills. If you have a living will and/or a durable power of attorney, please give copies of these documents to our receptionist to be included in your medical record. These documents would be forwarded with your records in the unlikely event that admission to a hospital is required. If you have any questions regarding our policies, please call our office.
